- Must be enrolled in the VA Puget Sound Health Care System and assigned a primary care physician |
- Retirees and eligible dependents enrolled in Medicare Parts A & B or found ineligible prior to April 1, 2001.
- Free at military treatment facilities.
- $3 per 90-day supply through the National Mail Order Pharmacy or TRICARE retail network stores ($9 for brand name drugs.).
- $9 or 20% (whichever is greater) of total costs for non-network pharmacies plus deductibles ($250/per person/$300 per family. |
