To submit an address and/or phone number change, please complete the appropriate forms and return them to the Human Resources Department or other organization as detailed below.
Regular employees making dependent and/or name changes, please refer to the Dependent Change page also as there are additional forms required.
Extra Hire employees needing to make a name change in addition to an address or phone number change, are required to submit a copy of their updated Social Security Card, along with the appropriate forms indicated below, in order to complete the name change process.
