Team Coaches/Individual Registration Forms

On the form list all of the available judges from you rink attending the ISI Spring Fever Team Competition. Please print their complete name, ISI Associate Member number, and ISI Judges Certification.

Sprinker will send every team coach a blank judges schedule, so that each judge can sign up for a judging slot when and where they are available. This will eliminate any conflicts and no-show judging assignments.

Send form to Sprinker Recreation Center by March 17, 2012.

Verification

The Team Coach or Rink Manager must sign each form to verify that the information contained on the entry form is true and correct. The information on the entry form must be accurate and complete. Inaccurate and/or incomplete forms will be returned. False information will result in disqualification. There will be a $25 fee for any changes made to your original entry form.
