    1. The purpose of this form is to collect information on residential structures that suffered damage from high winds, flooding, landslides, and/or mudslides between January 20 to February 10. Voluntarily submitting this information is not part of a claims process and does not guarantee reimbursement. Pierce County Department of Emergency Management will use this information to compile a preliminary assessment of all damages incurred throughout the county. You may be contacted at a later date if additional information is needed. You may print this form before pressing submit to keep for your personal records.
